Eligibility Check List
Please read the below eligibility check list before continuing to the referral form.
- I confirm that I have the right to live in the UK
- I confirm that I am currently able to claim benefits
- I confirm that I am able to claim housing benefit for myself
- I confirm that I have issues with substance / alcohol misuse
- I confirm that I am on 30mg of Methadone or 4mg of Subutex or less and will bring a 6-week bridging script to drop down to 0mg. (This only applies if you are on Methadone or Subutex)
- Please note that certain types of medications are not allowed at Reflections. E.g. Pregabalin, Codeine and Diazepam. This will be discussed in the assessment.
